Below, we have provided a high level view of what the role entails, with the complete job description discussed in detail during our interviewing process.


  • Provide clinical pharmacy input during the preparation of interdisciplinary care plans for patients.
  • Provide comprehensive pharmaceutical care for the network’s patients, per patient-specific referrals:
    • Review patient records, including allergies, problems, health maintenance, medications, and immunization status
    • Elicit and record updates to allergy and medication records
    • Perform patient assessment for medication-related issues
    • Identify any barriers to patient adherence
    • Interpret data related to medication safety and effectiveness
    • Provide information, education, and counseling to patients about medication-related care
    • Document all services in documentation platform and/or patient EHR in a timely manner
    • Participate in multidisciplinary reviews of patients’ progress
    • Communicate relevant issues to physicians and other healthcare team members Provide individualized health promotion and disease prevention interventions
  • Responsible for employees adhering to Standards of Operation, Core Values, and all other company policies.
  • Responsible for overseeing annual review process for positions that report to the Vice President – Clinical Compliance Officer.
  • The ability to provide training to positions that report to the Vice President – Clinical Compliance Officer on Company Standards of operations.
  • The ability to provide ongoing monitoring of standards by conducting standards testing on positions that report to the Vice President – Clinical Compliance Officer.
  • The ability to perform the physical, use of senses, cognitive, and environmental functions of the position, as specified on the physical demands.
  • The ability to build and maintain confidence and credibility with all clients and employees.
  • The ability to maintain friendly, cordial relations with all clients and employees; maintain a positive work atmosphere by acting and communicating in a manner that results in a positive work relationship with customers, co-workers, and managers.
  • Ability to comply with Company Standards of Operations. 
  • Ability to adhere to the Core Values of the Company, of teamwork, communication, empowerment, quality of care, and friendliness. 
  • Perform other duties as assigned by management

Experience Requirements and Preferences:


  • Doctorate of Pharmacy from a school of pharmacy accredited by the Accreditation Council for Pharmacy Education
  • Pharmacy residency accredited by the American Society of Health-System Pharmacists (community pharmacy, ambulatory care, geriatric pharmacy, or managed care pharmacy focus preferred)
  • Current, unrestricted pharmacist license in at least one state




  • Two to five years of recent CMM or MTM experience involving direct patient care

Current License or Certification:

  • Board certified via BPS or NBMTM
